Stanley DNA Release of Information ConsentThe Release of Information form, throughout this website referred to as the "Consent" form, is needed by the Project Administrators in order to publish a little more about participants in the DNA project. Without this form, the only information published will be your kit ID# and paternal ancestor chart. Each item on the form you want us to publish on this site has to be initialed by you. The signed form needs to be send to the address shown on the form by regular mail. Continue to the consent form. |
